JUF Right Start Program (new families only)
Below are the forms the Illinois Department of Children and Family Services (DCFS) requires for all children enrolled in a DCFS licensed program. You can access each form by clicking on the highlighted titles below. All forms must be received prior to the first day of attendance.
Medical Form: must be completed and signed by the child’s physician. Medical forms must be accompanied by proof of immunization and a lead risk assessment. A parent/guardian signature is required (there is a space on the right side of the form about one-third of the page down from the top)
Medication Consent Form: for any medication provided to the school, including EPI-pens
Emergency Card: state law requires that we have TWO contacts – other than parent(s) – for every child attending
Enrollment Form: completed by parent/guardian
Personal History: completed by parent/guardian
Permission Slip: each section signed and dated by parent/guardian
Facebook Permission: completed, initialed, signed and dated by parent/guardian
Discipline & Guidance: read policy; sign, date and return final page

Birth Certificate: provide a legible copy to be kept on file at school
